The film, a prequel to the 2002 film "Cabin Fever" and the third instalment in the series, revolves around a group of pals accidentally ending up being embroiled in a gruesome and fatal viral break out.

Plot Summary
The film starts with Porter (Sean Astin), a provider of a flesh-eating infection and the last survivor of a viral outbreak in the Caribbean, being kept in a suspicious medical research center. At the same time, a group of 3 pals, Marcus (Mitch Ryan), Dobbs (Ryan Donowho), and Josh (Brando Eaton), together with Josh's girlfriend Penny (Jillian Murray) prepare a bachelor party on a deserted island off the very same coast.

While Porter deals with Dr. Edwards (Currie Graham) in the upmarket laboratory's ominous designs, the celebration on the yacht turns south when they come across the human residues of the fatal outbreak. The crew finds Porter's brother's rotting corpse, and consequentially, Penny gets contaminated with the deadly infection after coming in contact with the blood.

Viral Outbreak and Escalation
As Penny's condition worsens, the group finds the secret research center while searching for help. The buddies concern the awful realization that they are on an island that served as the ground zero for the break out. The strength mounts as the group meets the ill-fated Porter and discovers the horrific reality of the research facility. Cent turns violent due to the impacts of the infection and attacks her partner Josh, who gets contaminated also.

Climax and Aftermath
Following a violent and gory series of incidents in the laboratory, including the massacre of the rest of the medical personnel, Porter manages to escape to the surface area after a fight with Dr. Edwards.
