The movie begins with the funeral service of Ellen Leigh, the secretive matriarch of the Graham family. Following her death, her child, Annie (Toni Collette), her spouse, Steve (Gabriel Byrne), and their two children, Peter (Alex Wolff) and Charlie (Milly Shapiro) manage the loss in various ways.

Annie, who is an artist, begins going to a sorrow support system unbeknownst to her household. There, she exposes that she had a complex relationship with her mom and that her household has a history of mental illness. This includes her father, who struggled with psychosis before committing suicide, and her brother, who hanged himself after accusing their mom of attempting to enact wicked on him. On the other hand, Charlie, whom Ellen was especially near, starts seeing her deceased granny's apparitions.

When Peter is designated to take Charlie to a party, disaster strikes as Charlie suffers an allergy, and in an unsuccessful effort to get her medical assistance, she is decapitated in a cars and truck accident. The family is ravaged by this horrific event, leaving Annie to blame Peter for the mishap.

Supernatural Discoveries
As the family starts to unwind, Annie begins seeing phantoms of her daughter and questions if she is going mad. In an attempt to deal with her grief, she goes back to the support group, where she meets Joan (Ann Dowd), who later on introduces her to the world of séances. Joan teaches Annie a routine to communicate with the dead, and Annie tries to call Charlie. After experiencing an eerie, supernatural existence, Annie thinks the routine works.

Annie begins observing unusual signs around her house and on her mom's belongings. As her fixation with the supernatural world grows, she discovers an image of Joan at her mother's funeral and understands that they knew each other. Later, Steve receives a call that Ellen's grave had actually been desecrated. This discovery leads Annie to investigate her mother's belongings further, where she discovers books about a demonic cult, along with a series of letters detailing a plan to "conjure" the demon Paimon using Annie's household.

Climactic Confrontation
Annie tries to encourage her family of her discoveries, but Steve stays hesitant, thinking that sorrow is impacting her mindset. Overwhelmed by the supernatural events occurring around them, Peter jumps out of his class window in a panic, injuring himself, but not fatally.

She chases her son, and after cornering him, signs of Annie's awareness emerge-- she cuts her own avoid with a wire, sacrificing herself to release Paimon. In the end, an army of cult fans welcomes Peter as the new version of Paimon, exposing that Joan, Ellen, and the cult had manipulated the whole series of events to bring Paimon into the mortal world.
