Plot Summary
The film begins with a selection of different individuals dying in a wide range of ways, from motorbike accidents to muggings gone wrong. They all of a sudden awaken in an apparently abandoned hospital with no concept how they arrived or how they endured their evident deaths. Led by a man named Sam, who passed away with his pregnant partner in an automobile mishap, this varied group begins to realize they might be in Purgatory, a place between life and death where they should confront their past sins before they can carry on to either Heaven or Hell.

Unveiling Secrets
As they browse the run-down passages of the hospital, they stumble upon frightening supernatural entities and experience macabre visions of their pasts that expose their deepest, darkest secrets. These visions force them to reassess their lives and challenge the dark truth they had actually evaded while alive. Each character experiences various degrees of inner chaos: John, for instance, is an alcoholic who killed his other half in an inebriated rage; Simon, a drug addict, killed his mother for money to buy drugs.

The Reaper's Game
They then meet 'The Reaper' played by William Foley, a demonic entity impersonating a guy of fabric. He informs them they remain in his 'kingdom' and not Purgatory, and they've been specifically chosen to play a wicked game. To survive the night and leave the hospital, they need to admit their sins, repent, and look for forgiveness before dawn, otherwise, they are doomed to eternal damnation. The Reaper enjoy their sufferings, continuously manipulating their emotions and enhancing their guilt, requiring them to relive their worst deeds.

Ending
The ending is a dramatic climax where Sam discovers he had actually committed an unspeakable act: he smothered his comatose better half with a pillow, unable to bear her suffering. In misery, he recognizes this as the true cause of their automobile mishap. The remainder of the group likewise confesses and repents their sins. Though some manage to find satisfaction in time, others, consisting of Sam, are far too late.
