Context matters: this is the Republican-populist critique of health care as misaligned incentives, often deployed to argue for market reforms, health savings accounts, or wellness programs rather than broad entitlement expansion. The line is persuasive because it contains a partial truth, sharpened into an accusation that invites the listener to see themselves as the responsible striver getting played.
