In late 19th-century Russia, medical care was uneven, treatments could be brutal or ineffective, and professional authority often traveled with paternalism. When outcomes are uncertain and power is lopsided, the line between help and harm blurs fast.

As a dramatist, he also understands timing and escalation. The joke works like a scene: set up the familiar villain (the lawyer), then reveal the darker twist (the doctor) to expose a broader cynicism about institutions. It’s comedy as diagnosis: laughter as the moment you realize the system can take everything - your savings, your agency, even your life - and still call itself service.
