Plot
Renee is a single mother living a mundane life in Kansas City with her child Evan. An arachnophobe, her life takes a drastic turn when she is abducted by a group of strangers after blowing a tire in her car. She wakes to discover herself bound and held captive in a scary, sanitized center together with other captives. It becomes clear that she's been abducted by a shadowy organization that conducts human testing with sinister purposes.

The strange group led by Dr. Nyman topics Renee to a series of fear-inducing explores the goal of triggering her 'rupture', a transformative development they claim is the next action in human evolution. The procedures focus on her inmost fears, that include spiders. It is exposed to Renee that she brings an uncommon genetic mutation that can supposedly activate this development when exposed to severe fear.

Defiance and Escape
Renee contradicts her fate as simply a lab rat. She shows to be a defiant and resourceful hostage, fighting back against her captors and staging several escape efforts. Eventually, she manages to break totally free and starts to look for an escape. Along her effort she encounters other slaves whom she attempts to free. Sadly, she also finds that the captives who had been successfully "ruptured" have all been changed into something disturbingly inhuman, revealing unusual physical characteristics and behavior.

Climax
The climax takes place as Renee, on her escape trail, discovers a room filled with spiders where she faces her deepest worry. Forced by her captors, while attempting to reach for a gun, she's subjected to a worry so severe that she finally 'ruptures'. Conclusion
